Town Hall Job Application - If you would like to apply for a job at Town Hall (whether it's with the Front Office, Maintenance Department, or the Police Department), you will need to fill out this Job Application.
Police Department Job Application - If you are applying for a job with the Police Department, you will need to fill out this Job Application AND the Town Hall Job Application (above).
Marquee Form - We ask that you have all requests for weekend marquee postings to be to us by Thursday afternoon (the Thursday before the weekend you want your message on the sign). If Friday is a holiday, please send in your request on Wednesday. Also, try and keep your message to a minimum. If there are a lot of words, we will have to make the text smaller and the message might be unreadable. Also, if there are too many words, we will have to select what we deem the most important information and will post only that. Please note: As of 1 May, 2017, there will be a $5 charge for all non-resident marquee requests. You will have to either mail a check to us or stop by Town Hall. If you opt to mail it, make your check payable to "Town of Shalimar" and mail it to #2 Cherokee Road; Shalimar, FL 32579. Ensure that we know what message(s) your check is for by either writing it on the check or enclosing a marquee form with your check. There will be no refunds for equipment failure or misspellings of your messages but we will post/repost/fix your message as soon as we are able. As of 1 June, 2017, we will no longer be posting personal pictures on the sign, it is MESSAGES ONLY.
House Watch Form - If you have plans to go out of town, feel free to complete this House Watch Form and our Police Department will do extra patrols in your neighborhood while you are away.
Resident Contact Update Form - If you are new to the Town or just recently changed your phone number or email address, please update your information with us using this form.
Contractor Tracking Certificate (CTC) Application - This application needs to be completed by all Contractors who have a job in the Town Limits (view Town Limit Map here). CTCs are valid from 1 October through 30 September. The cost is $20 for the year, $10 after 1 April. Along with this application, we will need your: 1) Current Workman's Compensation Insurance, 2) Current Liability Insurance, 3) Current State License (if applicable) and 4) Current Okaloosa County Business Tax or Competency Certificate. The CTC application can be filled in online, but you will still need to print it, sign it and bring it by Town Hall, email it to us (clerk@shalimarflorida.org), or (snail) mail it to us. If you decide to email your application, be sure to include all other required documentation. Also, you will need to (snail) mail us a check. Building Permit Application - If you have hired a contractor to have work done on your home or are doing repair work yourself, you will almost definitely need to come to us for a Building Permit before you get a permit with Okaloosa County Growth Management (County requirements for working within the Town Limits can be found here). This Application will also be used for Cable Installation/Maintenance and as a Sign Permit (temporary AND permanent signs). Please see our Building Permit Fee Chart to figure out your fee (it is based on the cost of the work being done). The Building Permit Application can be filled in online, but you will still need to print it, sign it and bring it by Town Hall, email it to us (clerk@shalimarflorida.org), or (snail) mail it to us. Business Tax Application - If you have a business that is located in the Town Limits of Shalimar, you are required to pay a Business Tax. Please see our Business Tax Fee Chart to figure out your fee. The Business Tax application can be filled in online, but you will still need to print it, sign it and bring it by Town Hall, email it to us (clerk@shalimarflorida.org), or (snail) mail it to us. Combs Park Reservation Form - The Pavillion at Combs Park is available to reserve for parties and other get-togethers. There is a $40 deposit for use of the pavillion but if everything is left as you found it, you will receive your deposit back to you. This Combs Park Reservation Form can be filled in online, but you will need to print it, sign it and drop it off at Town Hall, email it to us (clerk@shalimarflorida.org), or (snail) mail it to us. If you decide to email the form to us, you will still need to (snail) mail us a check. Make your check payable to "Town of Shalimar" and mail it to #2 Cherokee Road; Shalimar, FL 32579. Before submitting the form, you will need to call the Town Hall ((850) 651-5723) as we'll need to check our calendar to make sure the date you are requesting is available.
Conference Room Reservation Form - The Conference Room at the Town Hall is available to reserve for parties, meetings and other get-togethers. It is completely free to use if you are a resident of the Town. If you do not live in the Town, you can still use the room but there is a $25 fee. This Conference Room Reservation Form can be filled in online, but you will need to print it, sign it and drop it off at Town Hall, email it to us (clerk@shalimarflorida.org), or (snail) mail it to us. Code Enforcement Complaint Form - If you know of any Code Enorcement Violations in the Town, you can either call or email the Town Administrator directly (tb@shalimarflorida.org) or you can fill this form out and email it to him.
Equipment Hold Harmless Form - Almost everything the Town owns can be borrowed for free if you are a resident. The Maintenance Department has live animal traps, utility trailers, a tractor, a golf cart, a pickup truck, and numerous tools that can be rented out but you are required to complete this Hold Harmless Form. Before you submit the form, you will need to contact Town Hall (call, email or drop by) to make sure the equipment you want to borrow will be available when you need it.
